ACCESS Affordable Care Act Navigator and Women's Health Coordinator will educate and provide information on ACCESS' services and programs including primary and preventive care, educate on making ACCESS a medical home, help participants apply for health insurance through the Affordable Care Act, and provide information about breast and cervical cancer and issue referrals for mammogram and/or pap smears.
