VIDEO:  Integrated Health Home Pilot - Panel Discussion

At ACCESS’ annual employee meeting in October of 2018, ACCESS hosted its first panel discussion on our Integrated Health Home pilot with community partners, Catholic Charities and Trilogy Behavioral Healthcare.  During the panel, we explored the challenges and benefits of working together on our Integrated Health Home model.
