Jacob Meyer, M.D.

Dr. Jacob Meyer is a general Obstetrician and Gynecologist that focuses on minimally invasive gynecologic surgery and obstetrics. Dr. Meyer received his medical degree from Oregon Health and Science University and completed a Family Medicine residency at Contra Costa Regional Medical Center in California and, later, a residency in Obstetrics and Gynecology at Oregon Health and Science University.   Dr. Meyer has a focused practice designation from the American Board of Obstetrics and Gynecology in minimally invasive surgery. His surgical focus is on laparoscopic hysterectomy, vaginal hysterectomy, laparoscopic myomectomy, advanced hysteroscopy, pelvic organ prolapse surgery, stress incontinence surgery as well as recently branching out into V-NOTES and robotic surgery. He also has experience in high-risk obstetrics, taking care of patients with type 2 diabetes, hypertension, advanced maternal age, and multiple C-sections.   Dr. Meyer is enthusiastic about providing high quality medical care to all, regardless of their insurance status, and is dedicated to engaging patients in their birth plans so they have the birth experience they prefer. He is also passionate about empowering women to take control of their reproductive health and will collaborate with them to find the birth control method that is right for them.   In his spare time, Dr. Meyer enjoys spending time with his wife and two kids, coaching t-ball, running, going to White Sox and Bulls games, fishing, camping, and traveling. Dr. Meyer is fluent in Spanish.

Christopher Demers, C.N.M., W.H.N.P.

Chris Demers received their Master of Science in Nurse Midwifery at Georgetown University and completed their rotations at Northshore Health Systems in Lake Station, Indiana. Chris is certified through the American Midwifery Certification Board and the National Certification Corporation. They are a member of the American College of Nurse Midwives and the National Association of Nurse Practitioners in Women's Health. Chris chose to work at a community health center to provide patient-centered and holistic care within their community. Their philosophy of care centers on the belief that providers give patients the tools they need to make their own decisions. Chris enjoys cooking, going to the beach, and spending time with their cats.

Tania Nunez, L.C.P.C.

Tania Nunez earned her Master of Science in Community Counseling from National Louis University and completed her practicum at Rainbow House in Little Village working with survivors of domestic violence. She is credentialed through the Illinois Department of Financial and Professional Regulation and is a member of the Illinois Counseling Association and the Eye Movement Desensitization Reprocessing International Association. Tania chose to serve in community health after witnessing health disparities within her own community growing up. She works alongside her patients to ensure they are reaching their health goals and provides the best level of care to address their needs. Outside of work, Tania enjoys reading, listening to music, and connecting with her community. She is fluent in Spanish.
