Andres Mafla, M.D.
Dr. Andres Mafla earned his medical degree from Universidad del Valle in Cali, Columbia and completed his residency at Saint Francis Hospital in Evanston, Illinois. Dr. Mafla is a member of the American College of Physicians and has been an internal medicine specialist at Access Community Health Network for twelve years. Among his leadership roles at ACCESS, Dr. Mafla serves as a Regional Medical Director and is Chair of the Chronic Disease Committee. For the past three years, Dr. Mafla worked with the committee to improve ACCESS’s colorectal cancer screening rate from 13% to 45%. Through his leadership and diligent work on improving company policies, clinical workflows and changes to our electronic medical record (Epic), ACCESS has been able to significantly impact patient outcomes and develop best practices.
Chelsea Schroeder-Lozada, D.N.P.
Chelsea Schroeder-Lozada earned her Doctor of Nursing Practice degree from the University of Michigan. She is credentialed through the American Nurses Credentialing Center and is a member of the American Nurses Association, Michigan Council of Nurse Practice, and the Illinois Society of Advanced Practice Nursing. Chelsea is passionate about partnering with communities to improve health outcomes and advance health equity. She is dedicated to providing compassionate, patient-centered care that empowers her patients to take an active role in their health. Additionally, she is deeply interested in global health and completed her doctoral project within HIV positive communities in the Dominican Republic. When not at work, Chelsea enjoys spending time with friends and family, reading, and traveling.
Louise Judge, P.A.
Louise Judge graduated with her Masters in Physician Assistant Studies from Touro University Nevada. She is credentialed through the National Commission on Certification of Physician Assistants and is a member of the American Academy of Physician Assistants. She enjoys working with a diverse group of people and giving back to her community. In her practice, she works to meet her patients where they are, to provide high quality, holistic care. When not at work, Louise can be found hiking, traveling, and trying new recipes.
Yahaira Toruno, L.C.P.C
Yahaira Toruno holds a Master of Science in Rehabilitation Counseling and is licensed through the Commission of Rehabilitation Counselor Certification. As a licensed clinical professional counselor, Yahaira is dedicated to empowering patients who are struggling with chronic mental illness, substance use disorder, depression, and anxiety. She is here to support patients in whatever way they need and gives her patients the tools they need to make decisions that improve their health and well-being. Outside of work, Yahaira enjoys yoga, boxing, dancing, and science. She is fluent in French and Spanish.
Aaima Sayed, M.D.
Aaima Sayed received her medical degree from Stritch School of Medicine at Loyola University and completed her residency at Loyola University Medical Center. She is licensed through the Illinois Department of Financial and Professional Regulation. Dr. Sayed decided to work in a community health center to reduce the barriers for patients seeking behavioral health care services. She is a compassionate psychiatrist and always keeps her patients' best interests in mind. When not at work, Dr. Sayed enjoys spending time with her friends and family, trying new restaurants, and binging her favorite shows on Netflix. She is fluent in Urdu.
Kwang Jang, D.O.
Dr. Jang received his medical degree from the New York College of Osteopathic Medicine and completed his residency at the University of Chicago NorthShore. Dr. Jang is a member of the American College of Physicians and the American Osteopathic Association. As a scholar of the National Health Service Corps, Dr. Jang believes it is imperative to provide community health care for all who need it. He is committed to listening to his patients and to addressing their needs in a comprehensive manner. Dr. Jang speaks fluent Korean and when not working, he enjoys hiking, traveling, playing ping-pong, badminton and tennis.
Lisa Oldham, M.D.
Lisa Oldham earned her medical degree at Washington University, St. Louis and completed her residency at Rush University Medical Center. Dr. Oldham is Board-certified through the American Board of Obsterics and Gynecology and is a member of the American Medical Association, American College of Obsterics and Gynecology, and the North American Society of Pediatric and Adolescent Gynecology. Dr. Oldham was raised by civil rights activists and mentored by community physicians who taught her the importance of health equity and community involvement. She is committed to providing high quality, equitable care to all patients regardless of their background and looks forward to giving back to her community. Outside of work, Dr Oldam enjoys staying active, cooking, reading, and traveling.
Luiz C. Barbosa, L.C.S.W.
Luiz Barbosa received his Masters in Social Work at the University of Illinois-Chicago after completing his internship at the Catholic Charities of Chicago. He is licensed through the National Association of Social Workers and continues to be an active member. Luiz has a long history of working in cross-cultural settings and decided to work in a community health center setting to empower and support his community. His philosophy of care includes working to enhance and empower the well-being of his clients by meeting their basic and complex needs. Luiz is also interested in cultural events and philosophy. He is fluent in Portuguese and Spanish.
Frances Locke Rosenberg, C.N.M
Fran Locke Rosenberg is a graduate of Yale University and of the University of Illinois at Chicago and is a member of the American College of Nurse-Midwives. Originally from Troy, New York, Fran is married with four children and enjoys cooking hiking and spending time with her family.

Toree Stalter earned her Master of Medical Science in Physician Assistant Studies at the University of the Cumberlands Northern Kentucky Campus. She is Board-certified through the National Commission on Certification of Physician Assistants. Toree views herself as an advocate for her patients and works to provide affordable and accessible services in their own communities. She understands the importance of creating a safe space and building positive, trusting relationships with her patients. Toree also enjoys reading, cooking, and spending time with loved ones.
Roseline Nwokeji, A.P.R.N., F.N.P.-B.C.
Roseline Nwokeji received her Master of Science in Nursing from Olivet Nazarene University. Roseline is credentialed by the American Nurses Credentialing Center, and a member of the American Association of Nurse Practitioners and the Sigma Theta Tau honor society. Roseline is drawn to community health and preventative care after seeing too many cases of preventable illnesses during her time as an R.N. in an intensive-care unit. She believes in empowering people to better care for themselves holistically. Outside of work, Roseline enjoys traveling and she is fluent in Ibo.

Ahmed Memon graduated from Walden University. Ahmed joined ACCESS serve communities and provide care regardless of race, color, religion or origin. In addition to English, Ahmed speaks Urdu, Sindhi and Baluchi fluently.
Nadira Ali-khan, M.D.
Dr. Ali-Khan earned her medical degree in India and completed her residency in Chicago. She practices with a holistic approach and focuses on educating the patient on prevention. Doctor Ali-Khan is fluent in Urdu and outside of work enjoys reading and staying current on advancements in medicine.
